If you or your child would like to signup for our riding program you must submit these 3 forms prior to riding at Dreams Go On. Please print these forms and mail them to us when you and your physician have completed them:
Dreams
Go On is a therapeutic horseback riding program for riders that are
physically & mentally challenged. We are staffed mainly with volunteers and
can offer programming based on the number of volunteers that we have.
Because of this we have a certain number of slots for riders. Application to
the program doesn't guarantee acceptance. We do maintain a waiting list and
we reserve the right to select riders who will gain the greatest benefit and
can also be safely assisted.
Riders must be:
- Seizure free for a year
- Weigh less than 160 lbs.
- Have the ability to be safely seated on a horse
This form authorizes your physician/caregiver to release records to us, but is not required by us:
